We are located in the Guardian Building across the street from Park Ave Bakery.
Look for the tan and green building with a ramp.
You can enter Helena City Parking Lot 9 next to the Base Camp.
There are 8 patient parking spots directly in front of the building.
Appointment Instructions
- Please arrive on time
- New patients will need to arrive 15 minutes early.
- Please bring current copy of your insurance card along with a picture identification.
- Bring a list of all current medications & allergies to your visit.
- If the patient is a Minor, a Parent/Guardian must accompany the Minor. A “Consent to Treat a Minor” form can be filled out for future visits. Please inquire at Front Desk.
